INSTRUCTIONS FOR FILLING OUT A DENTAL PROFESSIONALS, INC. TIMESHEET


1) Please take the time to fill out your timesheet legibly and include all pertinent information. We do have records of your address and social security on file, but including this info on your timesheets ensures quick and accurate processing of your pay. Fill out the date, start, and stop time as indicated in the example below. Remember to round time worked to the nearest five minutes (if you arrive at 8:13 write 8:15). If you want to calculate the hours you worked use the TOTAL boxes under the days worked as in the example. DO NOT WRITE in the TOTAL HOURS box marked with an asterisk, this box is used by the Dental Professionals office for verification purposes. Do not write directions, phone numbers, or other notes on the timesheet.
The more complete and easy to read a timesheet is, the faster payroll is processed!

2) There are two types of work order numbers for jobs from Dental Professionals. If your work order number begins with an L, as in LA002 or LD030, your check will come from the Dental Professionals headquarters. If the job you accept has a number with a B, as in BH001, the office you temp at will furnish a check for you. By law the office you temp at has thirty days to pay you. In some cases, you may not be paid the same day you worked! Payroll is processed biweekly at Dental Professionals; please refer to the payroll schedule enclosed with your timesheet packets and checks. The top two copies of the timesheet, the white and yellow sheets, are sent to Dental Professionals.

Summary of Work Order Numbers:
L- Check comes from Dental Professionals.
B- Check comes from the office you worked at.
ALL TIMESHEETS, L and B jobs, SHOULD BE SENT IN WEEKLY!
Dental Professionals is unable to bill clients or pay you until timesheets are received (with an employer signature) in our office. It is not the clients' job to mail your timesheets, please do not depend on others to get your timesheets in. It is your responsibility to get timesheets to our office on time!

 

Where to turn in timesheets after hours

Timesheets can be left, after hours, in a drop box. The drop box is located in the underground garage accessible from 42nd Street (near the clock tower).
Walking: From the Clock tower, enter the garage the and turn right. The box is white with a Dental Professionals Logo on the front and is attached to the wall between the walkway entrance to the garage and the attendant's booth.
Driving: Go into the underground garage entrance accessible from 42nd Street (near the clock tower) and turn left after the attendant booth. The box is on your left, attached to the wall between the attendant's booth and the doors to the building. The box is white and has our Dental Professionals logo on the front.
From inside the building: Go to the 2nd floor and go straight out the doors into the garage. Walk straight ahead toward the attendants booth. The box will be attached to the wall on your right and is white with a Dental Professionals Logo on the front. It is located between the walkway entrance to the garage and the attendants booth.
