We have placed our procedures and documents on this web site so you can review them when needed. You are currently listed with Dental Professionals to assist you in finding permanent and/or temporary employment. If you have questions regarding any of our policies please contact us directly.

TheTerms of Referral pertains to permanent referrals only. If you are registered only for permanent placement and wish to change your status to include temporary placement we will need to meet with you personally. 

You will need to provide the following documents at both your orientation meeting with us, and with any dental office that pays you directly:           

All temporary professionals need to bring the following to our office only:

• Current Resume
• CPR/BLS Card (optional)
• Current TB Test within the last year (optional)

If you are working as a temporary or on a working tryout you should review our Office Policies and Payroll Info. In order for us to give you temporary work, you must sign that you have had your Hepatitis B vaccination or that you refuse to have it. Our Hepatitis B vaccination is available on our Forms page. Our Forms page also includes other important forms as well as links to Bloodborne Pathogen training which all dentists, dental hygienists, and dental assistants are required to complete annually. Please review the instructions on the BBP Training Instructions to complete this training.
